Medical fit-outs: clinical standards, built in

GP clinics, dental surgeries, allied health, pharmacies — medical tenancies carry standards general shopfitters rarely meet: clinical zoning, sterilisation rooms, accessibility, drug storage and infection control. Here's the full picture, state by state.

The build

What a medical fit-out actually contains

A clinic is a small hospital: zoning, hygiene surfaces, services redundancy and access standards all apply.

Clinical zoning

Clean/dirty flow separation: reception → consult → treatment → sterilisation → waste. RACGP and dental board guidance expect logical patient and instrument flows.

Sterilisation & reprocessing

Dental and surgical tenancies need dedicated sterilisation rooms: bench-mounted autoclaves with clearance, dirty-to-clean bench flow, dedicated hand basins, compliant ventilation.

Hygiene surfaces

Seamless vinyl coved at walls, washable ceilings in clinical rooms, non-porous benchtops. Every junction detail is an infection-control decision.

Accessibility — AS 1428

Accessible entry, path of travel, consultation rooms, ambulant and accessible WCs. Healthcare premises are public buildings — access compliance is assessed at approval and again at registration.

Drug storage & security

Drugs of dependence safes (anchored, alarmed per state pharmacy/poisons rules), secure vaccine refrigeration with monitoring, controlled-substance storage signing-off with the state authority.

Services & compliance

Medical gas rough-ins where applicable, redundant power considerations, X-ray shielding (with radiology approvals), increased ventilation, and close-out: certificate + practice registration.

Costs

Medical cost benchmarks 2026

Indicative, excluding GST. Sterilisation scope, shielding and services upgrades drive the spread.

Type$/m²What drives it
Allied health (physio, psych, consult rooms)$1,400–2,200Consult rooms, accessible amenities, modest services.
GP clinic$1,800–3,000Treatment rooms, infection-control surfaces, vaccine storage, access compliance.
Dental surgery$2,500–4,000+Sterilisation suite, chair plumbing rough-ins, X-ray shielding, compressor rooms.
Day surgery / specialist$4,000+Licensing-grade clinical fit-out, medical gases, theatre-grade ventilation.

State multipliers apply — the calculator covers office-grade baselines; clinical scope needs a medical-experienced shopfitter's estimate.

State rules for medical fit-outs

Questions

Medical fit-out FAQs

Why do medical fit-outs cost more than retail?
Three adders: clinical materials and detailing (coved vinyl, washable everything), services depth (extra plumbing, ventilation, shielding, safes), and compliance documentation across building, radiation and health authorities. The room count looks similar to retail — the content is not.
Can I fit out before accreditation?
Build first, accredit against the finished premises. But design to the standards from day one — RACGP/ADA accreditation inspectors check room sizes, flows and amenities that cannot be cheaply changed later.
How long does the approval stack take?
Change-of-use DAs run 2–4 months in most jurisdictions; radiation equipment approvals run parallel; building approval 2–6 weeks. Start the whole package before you commit to a fit-out contract with a fixed opening date.
What about lease make-good when we leave?
Medical fit-outs are expensive to remove. Negotiate the make-good clause before signing — many landlords accept "leave in place" for clinical improvements, which can save six figures at exit.
Can any shopfitter build a clinic?
No — and this is where directories earn their keep. Sterilisation flow, shielding coordination and infection-control detailing need contractors who've done clinical work before. Ask for two completed medical references minimum.
